Quest Diagnostics Data Breach Investigation

Chimicles Schwartz Kriner & Donaldson-Smith LLP is investigating potential class action claims against Quest Diagnostics Incorporated following disclosures that an unauthorized third party gained access to sensitive patient records stored within its digital provider network.

Quest Diagnostics is one of the nation’s largest clinical diagnostic laboratory operators, providing testing services across the United States through hospital systems, independent physician offices, and thousands of patient service centers.

What Happened at Quest Diagnostics?

According to a Form 8-K filing submitted to the U.S. Securities and Exchange Commission (SEC) on October 22, 2026, Quest discovered on October 21, 2026, that an unauthorized third party accessed its Quanum LSM (Lab Solutions Management) system.

Quanum LSM is a proprietary electronic portal utilized by healthcare providers, clinics, and commercial healthcare accounts to place diagnostic test orders, receive test results, and manage patient files. While Quest stated that the incident has not materially disrupted clinical testing or general operations, unauthorized access to the portal potentially exposed sensitive patient records.

Based on the functionality of the Quanum platform, the information potentially accessible may include:

  • Full names
  • Dates of birth
  • Contact information (addresses, phone numbers)
  • Diagnostic test order details and laboratory results
  • Healthcare provider and clinical facility identifiers
  • Patient account and insurance identification numbers

Quest stated that it notified federal law enforcement and engaged third-party cybersecurity forensics firms to investigate the incident. The company’s initial public filing did not disclose the total number of individuals potentially affected.

Why Is This Important?

The compromise of protected health information (PHI) and clinical records presents unique and severe risks that extend beyond standard financial identity theft. Unlike credit card numbers, sensitive medical histories and diagnostic test results cannot be cancelled or replaced once exposed.
